Flu vaccination events in the Atrium this week!

Occupational Health Services (OHS) will host numerous flu vaccination events where the following employees can get no-cost flu vaccinations with their employee ID badge:

• UF Health Shands employees and volunteers

• UF Health Physicians employees

• UF College of Medicine faculty physicians, residents and fellows

• Credentialed ARNPs

• Credentialed physician assistants

• Credentialed medical staff

These employees can also receive a no-cost flu vaccine at OHS. Just walk in to receive your free flu vaccine; no appointment is necessary. Click here for more information.

THE FOLLOWING VACCINATION EVENTS WILL BE HELD AT OUR ARCHER ROAD HOSPITALS THIS WEEK:

UF Health Shands Hospital Cancer Hospital (Room 1205)

• Monday, Oct. 24: 6:45 a.m. to 11 a.m.

UF Health Shands Hospital Atrium

• Wednesday, Oct. 26: 6:45 a.m. to 5 p.m.

• Thursday, Oct. 27: 6:45 a.m. to 5 p.m.

• Friday, Oct. 28: 6:45 a.m. to 5 p.m.*

*All events without an asterisk will be co-hosted by the UF Student Health Care Center (SHCC). When SHCC is on site, all UF faculty, staff and students can receive a no-cost flu vaccination. Those receiving vaccination through SHCC need to bring an insurance card in addition to their employee ID badge. For co-hosted events starting at 6:45 a.m., SHCC staff will arrive at 8 a.m.

DID YOU RECEIVE YOUR FLU VACCINE ELSEWHERE?

If you receive your 2016-17 influenza vaccination from your physician office, urgent care clinic, retail pharmacy or health department, proof of vaccination must be brought to OHS, or fax it to 352-265-1101.Your name, date of birth, employee ID # and date of vaccination must be included on the proof of vaccine. Incomplete forms will not be accepted. By providing this documentation, you will be counted as being vaccinated and will receive a sticker for your badge.

DECLINING VACCINATION

All UF Health Shands employees choosing to opt out of vaccination and wear a mask from Oct. 31 through April 30 must complete the 2016-17 Influenza Vaccination Declination module electronically via MyTrainingby using the Activity Search (on left side of screen) to find the module titled “Declination of Influenza Vaccination 2016-17. The module will be available beginning Oct. 1.
